Where is the coupon I was to receiver to send in for 50.00 rebate or something and I could get the modem free,?

When I signed up for internet and DSL a month ago the girl told me I would receive a $50.00 voucher to send in to get a rebate . I don't believe I ever received it and I got charged 49.95 on this bill which she told me a would. I would like this vocher or coupon to fill out.Where is the coupon I was to receiver to send in for 50.00 rebate or something and I could get the modem free,?
That's not good. Sometimes the reps you talk to will promise the moon just to get you to sign up.
